Sick Note For Less Than 7 Days

If you have been ill for 7 continuous days or less you do not require a doctor's sickness certificate.  Your employer may ask you to complete a self-certification form (SC2) which you can get from your employer or find one here.

Sick Note For More Than 7 Days

If you have been ill for more than 7 continuous days you will require a Fitness to Work certificate from your doctor (sometimes called 'fit note').  The doctor may call you or arrange for an appointment.  We normally send these out via link sent to your registered mobile phone but they can also be collected from the GP Surgery.

Additional Sick Note

If you have already had a fit note then contact us again the same way for a fit note for more than 7 days.  The Doctor may not need to see you to issue an additional Sick Note but this is the discretion of the Doctor.
